Under the direction of the Wellness Nurse Supervisor, performs at an advanced level, utilizing an interdisciplinary approach to care and services to residents in the independent living setting. Coordinates care with preferred providers or insurance providers. Collaborates with supervisor, other managers and directors to ensure resident needs are assessed appropriately for supportive services or a higher level of care. Provides preventative health and triage services to employees to include Tb skin testing, assessment for care of employees exhibiting signs of illness or in the event of an employee incident.
Major Duties and Responsibilities:
Responsible for completing health screenings for placement in any one of the independent or assisted living areas, or for preferred provider services if needs exceed what is safely provided in any one or our heath care licensed areas.
Follows up with residents in acute care or rehab settings to facilitate safe discharges back to Royal Oaks with appropriate services.
Manages the Med Assist Delivery program by ensuring the receipt of physician orders, communication with pharmacy, receiving medications from the pharmacy and the accurate and timely delivery of medications to the residents on service.
Expertly assists with unexpected or emergency nursing situations that are commonly seen throughout the facility. Maintains required documentation processes and procedures of such incidents.
Operates within the scope of practice of licensure per the Arizona State Board of Nursing
Knowledge and Skills:
Thorough knowledge of professional nursing practices/theories, techniques and tactics.
Excellent oral, written, presentation and interpersonal skills.
Computer proficiency-- Microsoft Word/Excel, Outlook and Internet.
Organized, able to multi-task, think creatively and adapt to different situations.
Education and Experience:
Minimum education level for RN: successful completion of AZ State Board of Nursing approved licensed practical nurse program
Minimum education level for RN: Bachelor Degree in Nursing
Preferably a four year degree from an accredited college or university in nursing, public health, or related field.
Preferably three to five years of progressive responsibility as a licensed or registered nurse in a health care environment; or a combination of education, training and experience that would ensure the effective execution of the duties of the job.
Current Arizona State Board of Nursing Registered Nurse Licensure.
Competency in the position maintained through continuing education
